The ACFP would like to formally congratulate the winner of the 2022 Patient’s Medical Home – Outstanding Family Practice award – the South Health Campus Family Medicine Teaching Clinic (SHCFMTC).
This award recognizes a clinic that exemplifies the vision of a Patient’s Medical Home (PMH) by going above and beyond to focus on the needs of patients and their communities. In January, the ACFP travelled to Calgary to deliver this award in person to the amazing team behind this clinic.
With team-based care at the heart of their operations, SHCFMTC consistently demonstrates what it means to be a patient’s medical home. The clinic is a medical home to nearly 7,000 patients, providing them with a high standard of care delivered by their multi-disciplinary team of physicians and allied health care providers. One of their many accomplishments include facilitating successful group-based education workshops delivered by multi-disciplinary teams on a variety of topics.
As a teaching site for the University of Calgary, SHCFMTC shapes the minds of future professionals by hosting 25 family medicine residents at the clinic for their two-year core residency program. Medical students, nursing students, and pharmacy students also rotate through the clinic for learning experiences that stay with them throughout their careers. By sharing their expertise and preparing the next generation of medical professionals, the future of family medicine is looking brighter than ever.
