The ACFP celebrated with its two Patient’s Medical Home – Outstanding Family Practice recipients and made a trip out to visit their staff teams. The ACFP’s Patient’s Medical Home – Outstanding Family Practice award honours clinics that exemplify the principles of the Patient’s Medical Home.
Dr. Margaret Churcher Family Practice, Calgary
The Dr. Margaret Churcher family practice office is team-based with care through a family physician, chronic disease nurse, pharmacist, shared mental health consultants, certified respiratory educator with assistance from an office manager, primary care coordinator, and MOA.
Wild Rose Medical Centre
The Wild Rose Medical Centre began in 1994 with three doctors. It now has 18 physicians having moved location to expand and accommodate needed physicians for the community. WRMC provides services throughout the community as well as at their principle clinic. WRMC holds clinics at the seniors care facilities and the local College. WRMC has been part of the Peak to Praries PCN since its inception and provide a dedicated perinatal care clinic with their supports in addition to in-clinic Chronic Disease Management counselling.
Last January, the ACFP stopped by in Olds to celebrate Wild Rose’s accomplishments. Special thanks to RhPAP for showcasing the visit.
