Identifying barriers and facilitators to person-centered integrated care for individuals with mild to moderate chronic kidney disease and multimorbidity

How is person-centered and integrated care for people with kidney disease and multimorbidity? Patients in Canada with chronic kidney disease and those that care for them have identified person-centered integrated care as a research priority area, but there is little research on this type of care for patients with mild to moderate chronic kidney disease and other health conditions. In 2023, we asked patients, caregivers, and healthcare providers from Alberta to evaluate their experience using a validated survey based on the Rainbow Model of Integrated Care. Participants identified some strengths, as well as gaps.
Now, we are looking to learn more about the barriers and enablers to providing person-centered integrated care for this population. We would like to invite healthcare providers (primary care physicians, nephrologists, nurses, pharmacists, dietitians, etc.) and administrators who provide care for patients with early-stage kidney disease and multi-morbidity to participate in a 30-minute telephone interview. A $50 gift card will be provided to compensate you for your time.
We are also inviting people with kidney disease (non-dialysis, non-transplant) and multimorbidity, as well as their caregivers, to participate in interviews. Participants must be over the age of 18 and located in Alberta.
