Are you interested in improving person-centered integrated care (PC-IC) for people with mild to moderate chronic kidney disease (CKD) and multimorbidity?
This study explores the barriers and facilitators to providing person-centered integrated care (PC-IC) for individuals in Alberta with mild to moderate chronic kidney disease (CKD, Categories G1–G3) and multimorbidity in both primary and nephrology care settings. Building on previous work using the Rainbow Model of Integrated Care Measurement Tool and qualitative interviews, this research uses a modified-Delphi approach to identify the most pressing challenges to PC-IC.
