Implementing Social Determinants of Health Screening in Primary Care in Alberta: The SPARK Tool Implementation Study

You are invited to participate in a pilot study testing the SPARK tool – an 18-question survey that collects patient demographic and social needs data.

This information helps care teams connect patients to community supports and tailor care, while also supporting quality improvement and program planning. The pilot will explore how best to implement SPARK in Alberta’s primary care settings, including integration into Electronic Health Records and effective patient engagement strategies.

The Ask:

      • Physician champions will support the one-year study by engaging their team, joining co-design and implementation planning sessions, and completing two brief surveys and interviews.
      • Physician champions will receive a $1,500 honorarium, and clinics will get $500 to support admin costs. For any workshop attended outside of work hours, a $50 gift card will be provided as a thank-you.

    The study is led by family physician Dr. Kerry McBrien and Dr. Stephanie Garies. It has received ethics approval (REB24-076).
