People with multimorbidity have complex care needs and high health care resource use. PC-IC is an effective approach to improve the management and outcomes for people with CKD. However, care can be fragmented and propose challenges at the early stages of CKD. This study builds on our previous work, where we systematically identified the challenges and gaps in PC-IC from the perspectives of patients, caregivers, and healthcare providers.
This study aim to identify the strategies used to provide PC-IC for this population through a 15-minute online survey. Participants will be offered a $25 gift card upon completion of the survey.
