During the month of November, members of the ACFP staff visited two clinics who demonstrate the pillars of the Patient’s Medical Home (PMH). Both clinics continue to grow (in size and staff) and provide similar advice when it comes to adopting the PMH model; look to the PMH clinics you admire for an initial framework and tailor the principles of the PMH to the unique needs of your community.

Bonnyville Medical Clinic, Bonnyville
Bonnyville Medical Clinic is a rural multidisciplinary clinic consisting of 16 full-time family physicians, six part-time family physicians, one internist, and three nurse practitioners. Their staff of 18 are also closely supported by the Bonnyville PCN and their group of nine nursing staff. Bonnyville Medical Clinic aims to be a rural centre of excellence for their community. Staff, students, and residents pride themselves on being a standard bearer for the Patient’s Medical Home and Rural Medicine!
Westgrove Clinic, Spruce Grove
Westgrove Clinic is a long-established Family Practice based in Spruce Grove that provides comprehensive cradle to grave care. They are early adopters aspiring to the medical home model of care with a multi-disciplinary team consisting of NP, RN, LPN’s, Pharmacist’s, Social Worker, and Behavioral Health consultant.
