Are you interested in better understanding the health journeys of people living with dementia and their caregivers
Participating clinics are asked to identify dementia care dyads (a person living with dementia and their caregiver, both attending the same clinic), using either a manual or algorithm-based method provided by the research team. Clinics will also help link CPCSSN data to other health data sources, which involves brief orientation meetings and providing consent for data linkage. Clinics not already part of CPCSSN will be asked to join as sentinel practices through a short onboarding process. No patient recruitment or consent is required, as the study has received a waiver of patient consent.
