You are invited to participate in a brief online survey exploring how primary care providers in Alberta assess and diagnose dementia, including any challenges, guidelines, or referral pathways used in practice. Your insights will help inform the development of a tailored dementia care pathway that supports both providers and people living with dementia and their caregivers.
The survey will take approximately 15–25 minutes to complete.
