ACFP's MAPS Submission
Every Albertan Has Access to a Family Physician and a Primary Health Care Team by Implementing the Patient’s Medical Home
Modernizing Alberta’s Primary Health Care System (MAPS) Initiative
In December 2022, the ACFP
were among many primary health care interest holders to provide a MAPS submission for consideration. The ACFP’s submission came together through the work, consultation, and collaboration of many members and partners including the Strategic Committee for the Future of Family Medicine, our Membership Advisory Committee, and our First Five Years in Family Practice Committee.
The ACFP’s central recommendation in its MAPS submission is to ensure that every Albertan has access to a family physician and a primary health care team by implementing the Patient’s Medical Home. To achieve this, the submission outlines supporting recommendations that include:
- Funding the expansion of family medicine teams at the point of care
- Building community-based health authorities with engaged leadership, patient-based funding, and accountability
- Ensure the necessary access to data to assist in clinical care and quality improvement
